Please use the link above to view an exemplar completed Annual Review Report. The features of a well-completed report include
- whether the description of the child's functioning is still accurate;
- the progress the child has made in relation to the statement objectives
- the provision that is in place to meet the identified needs - and its effectiveness;
- targets for the year ahead (agreed after reviewing last year's targets).
- child's views, parents views, reports from support agencies, and transition plan (if appropriate) should be attached
- indicate whether the statement should be amended or ceased.
If you have any queries about the completion of a report, please do not hesitate to contact your school's Senior Education Officer for SEN who will be happy to advise.
Pupil's Contribution to Reviews - Resource Bank for Example Materials
At the Summer Term (2007) SENCO Network Meetings a checklist to support "Involving Children and Young People (C/YP) in meetings and Reviews" was presented. As part of the presentations, examples of ways of gaining pupils views were collected. These have now been made available in electronic form.
The example materials represent:
- A range suitable for age, ability and specific conditions
- Variations to match a pupil's personality and personal style
- Use of various media – written, photograph, mind map, powerpoint, PECs, Inclusive Writer, Boardmaker
We would like to thank all those who provided and prepared examples. Please feel free to continue to contribute examples to this resource bank and feedback the use of these materials and what works.

Involving C&YP in Meetings and Reviews Guidance Pack - A Supportive Checklist.
This checklist has been developed to break down each of the principles and promises in the Yorkshire and Humberside Regional Partnership Guidance into further practical points on involving children and young people (C&YP) in reviews and meetings.
It is aspirational and is part of the journey to increase C&YP participation.
As you are already involving C&YP at some level, this checklist is to help identify further steps you can take.
It is not an exhaustive list nor is it expected that it will be followed in its entirety. It is intended to stimulate ideas and creative approaches.
Some of these practical prompts may no be appropriate in some meetings or with some C&YP.
The points are not exclusive to the one principle or promise under which they are listed.
C&YP should be given the opportunity through the provision of good information to attend or not attend meetings in which they are the focus.
The points are designed to cover a range of meeting and review situations.
This work is consistent with and supportive of other work around SEN matters eg: Inclusion Charter Mark and Self-Evaluation Tool for SEN.

Speech and Language Therapy
Use this link to find the new Speech and Language Therapy referral form for the Bradford NHS Trust (not Airedale). This form replaces all previous forms. Please note this form should be returned to the Speech and Language Therapy Service directly, not Education Bradford.

Section 8 - SEN Funding
The Government has recommended that Local Authorities make provision for a greater range of SEN in mainstream schools without the need for a Statement, reduce the reliance on Statements of SEN and so reduce the associated paperwork and bureaucracy.
In turn Central Government recommended that Local Authorities provide money direct to schools so that schools can make decisions to support children with SEN at the earliest opportunity.
Education Bradford has now adopted this strategy so that schools will use their delegated budgets to meet the needs of pupils with SEN and so make decisions as early as possible about curriculum and support arrangements for children who experience barriers to their learning.
Education Bradford has redistributed money to schools through a formula and as a result a significant proportion of the funding (over 80% of the Statement budget - representing over £5million) for children with SEN can now be met from the school budget.
Education Bradford has done this to make sure that the money available is used in a more effective way and to enable schools to identify children's needs as soon as they arise and be in a position to give appropriate support.

Section 9 - SEN / Inclusion Policies
Special Education Needs School Evaluation Framework
Following work undertaken by the SEN Strategy Group an SEN Self-evaluation Framework have been developed. It is hoped that schools will find this framework a useful 'tool' when undertaking evaluations of the effectiveness of their school's SEN Policy and feeding this data and information into the Ofsted SEF process.
The information is presented in two formats; one in landscape layout which enables the user to record evidence against criterion statements and ascribe a score, the other version, in portrait layout, is a text only version allowing schools to use the criteria offered as prompts for their own in-house review procedures.
